Codeine

The medications on this page contain codeine phosphate or dihydrocodeine. Some medications also contain paracetamol. Codeine phosphate and dihydrocodeine are used briefly to treat pain which has not responded to over-the-counter (OTC) medications. The former is also used to treat diarrhoea not caused by toxic substances and for dry, painful coughs.

Codeine phosphate and dihydrocodeine are safe and effective for most people. Be sure to tell your doctor about any pre-existing medical conditions, and all current medications or supplements.

Codeine Phosphate

Active ingredient: codeine phosphate

Strength(s) available: 30 mg and 60 mg

Average dose: Up to 60 mg per dose, as per prescription instructions.

Additional Information:

What is codeine phosphate used for?

Codeine phosphate is mainly used as a painkiller. It is useful in cases when over-the-counter (OTC) medications have not been successful, but can only be used for short periods of time. Codeine phosphate is also used to treat diarrhoea and painful, dry coughs.

Can I take codeine phosphate if I am pregnant or breast feeding?

Codeine phosphate is not suitable for nursing mothers, as the drug can pass to the baby through the milk. If used in late pregnancy, codeine phosphate may cause the baby to experience drug withdrawal symptoms after birth. If you are pregnant, speak to a doctor before taking codeine phosphate.

Why should I use codeine phosphate on its own instead of a combination of codeine and paracetamol?

Codeine phosphate is more effective as a painkiller when used with paracetamol than when used alone. At least four hours must elapse between doses of each drug. If they are in separate tablets they can be alternated every two hours to provide more comprehensive coverage.

Dihydrocodeine

Active ingredient: dihydrocodeine

Strength(s) available:  30 mg

Average dose: 30 mg per dose, as per prescription instructions.

Additional Information:

Why must I avoid alcohol with dihydrocodeine?

Alcohol increases the chances of suffering side effects such as nausea, drowsiness, dizziness, or slow and shallow breathing.

What is dihydrocodeine prescribed for?

Dihydrocodeine is used as a painkiller for pain that does not respond to OTC medications. It is particularly effective for nerve pain. Dihydrocodeine is often prescribed for conditions including sciatica, shingles and Paget’s disease.

Does dihydrocodeine cause side effects?

Most people will not experience any side effects from taking dihydrocodeine. Some people may experience effects including, drowsiness, dizziness, nausea, constipation and mild stomach pain. You should not drive or use machinery if affected. You should seek medical attention if stomach pain becomes severe.

Co-codamol

Active ingredient: codeine and paracetamol

Strength(s) available: 30/500 mg (30 mg codeine and 500 mg paracetamol)

Average dose: Up to two tablets 60/1000 mg per dose, as per prescription instructions.

Additional Information:

Should I use effervescent or regular tablets?

Effervescent tablets are absorbed faster by the body than regular tablets. Soluble medication may relieve pain faster. However, effervescent tablets contain higher levels of sodium than regular tablets. If you have been told to reduce your sodium intake, consider regular tablets.

Does co codamol need to be stored in a particular way?

Always store medication in a cool, dry place. Temperatures over 25 degrees can cause medication to degrade. Keep the tablets in the original box with the patient information leaflet in case you need to refer to the instructions. Always keep medicines out of the reach of children and animals.

Kapake

Active ingredients: codeine and paracetamol

Strength(s) available: 15/500 mg and 30/500 mg (15 mg or 30 mg codeine and 500 mg paracetamol)

Average dose: 30/1000 mg or 60/1000 mg per dose, as per prescription instructions.

Additional Information:

Is it better to take codeine and paracetamol combined, rather than codeine only?

Codeine works more effectively when used with paracetamol than when used alone. This is because the drugs work to relieve pain in different ways. Opioids, such as codeine, work by attaching to the opiate receptors in the body, mimicking the body’s natural painkilling chemicals. Paracetamol is thought to work by blocking pain signals in the brain.

What should I do if I have taken too much Kapake?

If you have accidentally taken too much of this medication, you need to speak to a doctor straight away, even if you feel fine. Paracetamol overdose can cause severe liver damage, which may not be immediately obvious. Taking too much codeine can result in breathing difficulties or an altered heart rate. Take the pack with you so doctors know exactly what medication you have taken.

Solpadol

Active ingredients: codeine phosphate and paracetamol

Strength(s) available: 30/500 mg (30 mg codeine and 500 mg paracetamol)

Average dose: 30/500 mg or 60/1000 mg per dose, as per prescription instructions.

Additional Information:

Is Solpadol addictive?

All medicines containing codeine are potentially addictive if used for too long. Follow the prescriber’s instructions regarding how long you are to take the medication. If you take Solpadol, or any other medication containing codeine, for an extended period, you may experience withdrawal symptoms when you stop taking it.

Can Solpadol cause mood changes?

Solpadol can cause mood changes in some people. Codeine works by mimicking and replacing endorphins in the body. Endorphins are the body’s feel-good chemicals, so medications containing codeine may cause changes in mood ranging from a feeling of well-being to confusion or anxiety. This should pass when the medication wears off.

Co-dydramol

Active ingredients: dihydrocodeine and paracetamol

Strength(s) available: 10/500 mg (10 mg dihydrocodeine and 500 mg paracetamol)

Average dose: 10/500 mg or 20/1000 mg (one or two tablets) per dose, as per prescription instructions.

Available to buy today in packs of 100 tablets and 200 tablets.

Additional information:

Is co-dydramol suitable for everyone?

Co-dydramol is not suitable for everyone. Because it contains dihydrocodeine, co-dydramol should not be taken if you suffer from breathing problems, paralytic ileus, liver disease, or if you have had a recent head injury. Co-dydramol is not suitable for use by pregnant women or nursing mothers.

Can co-dydramol cause an allergic reaction?

Any drug can cause an allergic reaction. Signs of allergic reaction include hives, swelling of the lips, throat, tongue, and difficulty breathing. If you start to suffer any of these effects, go to your nearest doctor or hospital immediately. Take the pack of medication with you so doctors know exactly what medication you have taken.

Zapain

Active ingredient: codeine and paracetamol

Strength(s) available: 30/500 mgs (30 mg codeine and 500 mg paracetamol)

Average dose: 60/1000 mgs, two tablets per dose. Or as per prescription instructions.

Additional Information:

Are codeine and paracetamol safe to take with other medications?

Codeine and Paracetamol should not be taken with some other medications, including some antidepressants, antibiotics and anti-epileptic medications. Tell your prescriber about any medicines or supplements you take so the prescriber can make sure no adverse interactions will affect you.

Can I do anything to reduce the risk of side effects?

Most people will not experience side effects when taking Zapain. However, it is possible to reduce the risk of two of the more common side effects, nausea and constipation. If you suffer from nausea, try taking the medication with food. To minimise constipation, make sure you drink plenty of water throughout the day.
