Vagifem is a vaginal tablet that belongs to a class of medications known as hormone replacement therapy (HRT). Each tablet contains 10 micrograms of estradiol, a form of the natural female hormone oestrogen and comes with an applicator to help you safely and easily insert it into your vagina.
Estradiol in Vagifem is identical to the oestrogen naturally produced by the ovaries before menopause and is suitable for post-menopausal women who have not had a period for at least 12 months.
Because Vagifem is inserted directly into the vagina, it slowly releases oestrogen to the surrounding tissues to help relieve symptoms such as vaginal dryness and irritation. Vagifem may be a suitable form of HRT if you are experiencing localised menopause symptoms.
Vagifem is a type of HRT that’s used to treat specific symptoms of menopause in women. As women age and approach menopause, their bodies produce less oestrogen, leading to various physiological changes throughout the body, including hot flushes, night sweats, mood changes, and vaginal dryness.

Once Vagifem has been inserted, it slowly releases estradiol over time. Some people notice initial relief from their symptoms in as little as 2 to 4 weeks of daily use. However, it can take up to 3 months to feel the full benefits of HRT.
If your symptoms have not improved or have got worse after 12 weeks, contact your clinician for advice, as they may suggest an alternative form of HRT.
It takes several hours for Vagifem to fully dissolve. Sometimes, it can take up to 24 hours. Because it is a slow-release tablet, it is recommended to insert it at night before going to bed to help keep it in place.
Research shows that after 12 weeks, 39% of people given Vagifem 10 mcg had a vaginal pH of less than 5, compared with 21% of those given a placebo. Vagifem has very low systemic absorption. Studies lasting up to one year did not show an increased risk of endometrial hyperplasia or cancer, but the product information states that endometrial safety with repeated or longer-term use is uncertain. Vagifem is a vaginal tablet which is inserted directly inside the vagina. It is not a pill that you swallow. You can start using Vagifem on any day which is best for you. Before starting Vagifem, it is important that you let your clinician know about any prescribed, over-the-counter, or herbal medications you are taking. Because Vagifem is a local treatment in the vagina, it is unlikely to affect other medications, but it may affect other vaginally applied treatments.
Vagifem is only intended to be used by postmenopausal women. If you become pregnant, stop using Vagifem and speak to your doctor. You must not use Vagifem while breastfeeding. If you are unsure, speak to your doctor for more information.
For most women, no. After the initial two-week treatment period, the usual maintenance dose of Vagifem is one vaginal tablet twice a week, with at least three days between doses. Using it only once a week may not provide enough oestrogen to keep vaginal symptoms under control.
If your symptoms are well managed or you are experiencing side effects, speak to your clinician before reducing your dose. They can advise whether a different treatment schedule or an alternative vaginal oestrogen would be more suitable.
There is no evidence that Vagifem causes weight loss. Because it is a low-dose vaginal oestrogen that acts mainly in the vaginal tissues, very little of the medication is absorbed into the bloodstream, making it unlikely to affect your body weight.
If you notice unexpected weight loss while using Vagifem, it is unlikely to be caused by the medication. Speak to your GP if the weight loss is unexplained, persistent, or accompanied by other symptoms.
Vagifem helps replace the oestrogen lost after menopause, relieving symptoms such as vaginal dryness, soreness, itching, burning, and pain during sex. It can also improve urinary symptoms linked to vaginal atrophy, including urgency and discomfort when passing urine. Yes, although it is an uncommon side effect. Some women may develop vaginal thrush while using Vagifem, which can cause itching, soreness, redness, and a thick white discharge. Menopausal changes themselves can also increase the risk of infections, so thrush is not always caused by Vagifem. If you think you have thrush, seek advice from a clinician before starting treatment.
Not usually. If you develop thrush while using Vagifem, you can normally continue your treatment while the infection is treated, unless your clinician advises otherwise. If you experience severe irritation, recurrent thrush, or are unsure whether your symptoms are caused by thrush or another condition, speak to your healthcare provider for further assessment.
Vagifem is more likely to improve bladder symptoms than irritate the bladder. Restoring oestrogen to the vaginal and urinary tissues can help reduce urinary urgency, frequency, burning, and recurrent urinary tract infections caused by menopause. However, it can sometimes cause local irritation, such as burning or discomfort in the urinary or genital tract.
This usually goes away on its own as your body adjusts, but if bladder symptoms worsen or you develop pain, fever, or blood in your urine, seek medical advice.
Yes, vaginal bleeding is a common side effect affecting up to 1 in 10 people who use Vagifem. Light spotting can occasionally occur when starting Vagifem as the vaginal tissues recover, but any bleeding after menopause should always be assessed by a clinician.
Contact your GP promptly if bleeding is heavy, continues, occurs after sex, or starts after you have been using Vagifem without problems for some time.
Canesten is available as oral, external, and intravaginal treatments. Do not use a Canesten vaginal tablet or pessary at the same time as Vagifem unless a clinician or pharmacist advises you to, because the Canesten leaflet says not to use other vaginal products during treatment. Advice may differ for an oral capsule or external cream.
Store Vagifem at room temperature in its original packaging, away from excess heat and moisture, and out of the sight and reach of children. Do not use the tablets after the expiry date printed on the pack. There is no need to refrigerate Vagifem. Always follow the storage instructions provided in the Patient Information Leaflet.
